OhioRISE Care Coordinator

Provide family-centered care coordination for Ohio youth with complex behavioral health needs. Lead wraparound teams and connect services across homes, schools, communities, and providers.

Description

  • Build and organize teams according to family needs
  • Plan, lead, and facilitate child and family team meetings
  • Deliver care coordination in families’ homes, communities, or other preferred settings
  • Work with youth, families, natural supports, and providers to identify care coordination needs and services
  • Support youth across home, school, and community environments
  • Guide teams in defining needs, goals, and action steps
  • Promote youth and family voice and choice throughout coordinated care and services
  • Keep records needed to obtain and retain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Stay informed about available community resources
  • Attend required in-person and virtual training and supervision sessions, including travel as needed
  • Complete assigned training and maintain required certifications
  • Comply with confidentiality, professional conduct, and incident-reporting policies
  • Use databases and electronic health record systems to document client services
  • Maintain compliance with HIPAA, mandated-reporting obligations, and professional ethics
  • Travel throughout the assigned service area
  • Carry out other assigned responsibilities

Requirements

  • Three years of relevant children and family services experience with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher; relevant fields include behavioral health, child welfare, developmental disabilities, juvenile justice, and related public-sector or behavioral healthcare services
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience to support children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver’s license and ability to operate a motor vehicle
  • Appropriate automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume setting
  • Ability to work in an internet-based, automated office environment
  • Ability to conduct and participate in on-camera video meetings and calls
  • Minimum 100 Mbps high-speed internet connection
  • Ability to sit for extended periods while working at a computer and lift up to 15 pounds.
