Clinical Coordinator RN – Physician Practice

Coordinate clinical staff, patient care, and daily physician-practice operations for Lehigh Valley Health Network. The role supports EMR workflows, competency development, care transitions, and quality improvement.

Description

  • Lead clinical operations and patient engagement through visit coordination, triage, EMR tools, pre-visit planning, care-transition follow-up, and patient-centered initiatives.
  • Provide clinical and administrative coverage when the Director or Manager is unavailable.
  • Complete closing procedures, including reports, no-show follow-up, daily batching, and office closeout tasks.
  • Prepare daily charts and maintain paper and electronic medical records.
  • Seek policy guidance when needed and coordinate with other departments for additional resources.
  • Evaluate clinical staff competencies and coordinate remediation with Clinical Services.
  • Coordinate care across settings for an assigned population, ensuring labs, diagnostic studies, and referrals are completed and reviewed with patients.
  • Offer regular performance feedback and engage colleagues in developing quality-improvement workflows.
  • Promote staff accountability for following policies, procedures, and professional practice standards.
  • Apply relevant research to practice while supporting organizational and hospital objectives.

Requirements

  • Associate degree in nursing from an accredited nursing school.
  • Three years of recent clinical-setting experience.
  • Active Pennsylvania Registered Nurse license.
  • Ability to initiate, accept, and adapt to change in a healthcare environment.
  • Capacity to work independently, take initiative, and collaborate effectively.
  • Strong computer skills, including EMR systems and Microsoft Word, PowerPoint, and Excel.
  • Effective time-management skills.
  • Mature emotional intelligence.
  • Project-management skills.
  • Ability to lift and carry up to 25 pounds.
  • Ability to sit, stand, and use a keyboard frequently.
  • Patient-care duties may require kneeling, bending, squatting, and performing CPR.
