Clinical Coordinator RN – Physician Practice
Coordinate clinical staff, patient care, and daily physician-practice operations for Lehigh Valley Health Network. The role supports EMR workflows, competency development, care transitions, and quality improvement.
विवरण
- Lead clinical operations and patient engagement through visit coordination, triage, EMR tools, pre-visit planning, care-transition follow-up, and patient-centered initiatives.
- Provide clinical and administrative coverage when the Director or Manager is unavailable.
- Complete closing procedures, including reports, no-show follow-up, daily batching, and office closeout tasks.
- Prepare daily charts and maintain paper and electronic medical records.
- Seek policy guidance when needed and coordinate with other departments for additional resources.
- Evaluate clinical staff competencies and coordinate remediation with Clinical Services.
- Coordinate care across settings for an assigned population, ensuring labs, diagnostic studies, and referrals are completed and reviewed with patients.
- Offer regular performance feedback and engage colleagues in developing quality-improvement workflows.
- Promote staff accountability for following policies, procedures, and professional practice standards.
- Apply relevant research to practice while supporting organizational and hospital objectives.
आवश्यकताएँ
- Associate degree in nursing from an accredited nursing school.
- Three years of recent clinical-setting experience.
- Active Pennsylvania Registered Nurse license.
- Ability to initiate, accept, and adapt to change in a healthcare environment.
- Capacity to work independently, take initiative, and collaborate effectively.
- Strong computer skills, including EMR systems and Microsoft Word, PowerPoint, and Excel.
- Effective time-management skills.
- Mature emotional intelligence.
- Project-management skills.
- Ability to lift and carry up to 25 pounds.
- Ability to sit, stand, and use a keyboard frequently.
- Patient-care duties may require kneeling, bending, squatting, and performing CPR.
