OhioRISE Care Coordinator — Ohio

Coordinate OhioRISE services for young people with complex behavioral health needs. Facilitate family teams and community-based care through ISBH, an Ohio behavioral health organization.

विवरण

  • Build and organize teams defined by each family
  • Plan, lead, and facilitate meetings for young people and their identified teams
  • Deliver care coordination in families’ homes, communities, or preferred locations
  • Work with young people, families, natural supports, and service providers to identify care needs and services
  • Ensure youth and families have meaningful voice and choice in coordinated care and services
  • Maintain documentation needed to access and retain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Keep current knowledge of available community resources
  • Complete required in-person and virtual training and supervision
  • Travel throughout the assigned service area
  • Use databases and electronic health record systems to document client information accurately
  • Complete other duties as assigned

आवश्यकताएँ

  • Experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare: three years with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher
  • Knowledge of and experience with High-Fidelity Wraparound preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience supporting children and families experiencing serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver’s license and ability to operate a motor vehicle
  • Required automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment
  • Ability to work in an Internet-based, automated office environment
  • Ability to conduct and participate in on-camera video calls and meetings
  • Minimum 100 Mbps high-speed Internet connection
  • Ability to sit for extended periods and work at a computer
  • Ability to lift up to 15 pounds
  • Ability to attend required training and supervision, including travel
  • Ability to complete assigned training and maintain required certifications
  • Ability to maintain confidentiality, professionalism, and incident reporting standards
  • Ability to comply with HIPAA, mandated reporting requirements, and professional ethical guidelines
