Epic Healthy Planet Application Analyst

Configure and improve Epic Healthy Planet to support population health and value-based care programs. Help with ACO operations, quality reporting, risk adjustment, and healthcare workflow improvements.

Tavsif

  • Configure, support, and optimize Epic Healthy Planet applications and related value-based care workflows.
  • Act as a main application support contact for end users and stakeholders.
  • Maintain Managed Contracts, Rosters, Attribution, Registries, Data Marts, Metrics, and Dashboards.
  • Support ACO operations and population health programs through configuration, troubleshooting, and workflow improvements.
  • Investigate and resolve application, workflow, and system issues affecting functionality and reporting.
  • Take part in application design, build, testing, implementation, enhancements, and ongoing support.
  • Write and run test scripts, verify system changes, and coordinate with business stakeholders.
  • Oversee data imports, file loads, mapping, and validation.
  • Maintain healthcare quality measure sets, including CMS, HEDIS, and PCMH.
  • Configure and maintain HCC and CDPS models for risk adjustment and population health programs.
  • Work with operational, clinical, and technical teams to identify process improvements and refine workflows.
  • Coordinate with internal stakeholders and external vendors to resolve issues and assess functionality and enhancements.
  • Help deliver annual quality measure updates and meet regulatory requirements.
  • Maintain system documentation, workflows, procedures, and support materials.
  • Track system performance and identify ways to improve efficiency, reporting, and end-user adoption.
  • Carry out other assigned duties and responsibilities.

Talablar

  • Strong practical knowledge of Epic Healthy Planet and population health workflows.
  • Understanding of value-based care, accountable care organizations (ACOs), population health management, and quality improvement.
  • Experience supporting Managed Contracts, Rosters, Attribution, Registries, Data Marts, Metrics, and Dashboards.
  • Strong understanding of healthcare quality measures and CMS, HEDIS, and PCMH reporting requirements.
  • Knowledge of risk adjustment methods, including HCC and CDPS models.
  • Ability to analyze and troubleshoot complex application, reporting, and workflow issues.
  • Experience with application configuration, maintenance, testing, upgrades, and optimization.
  • Strong analytical, critical thinking, and problem-solving abilities.
  • Excellent written and spoken communication, including explaining technical information to nontechnical audiences.
  • Strong interpersonal and active listening skills.
  • Ability to gather business requirements and translate them into technical solutions.
  • Ability to collaborate across teams while managing multiple priorities.
  • Careful attention to detail and commitment to accuracy, quality, and continuous improvement.
  • Excellent organizational and project management skills.
  • At least 2–3 years of experience supporting Epic Healthy Planet applications.
  • Current EpicCare Ambulatory Certification.
  • Experience supporting ACO operations, population health programs, and value-based care initiatives.
  • Experience with healthcare quality measures, including CMS, HEDIS, and PCMH.
  • Experience with data management, data mapping, file imports, and system configuration.
  • Knowledge of healthcare information systems and industry best practices.
  • Bachelor’s degree in Information Technology, Healthcare Administration, Health Informatics, Business Administration, or a related field, or an equivalent combination of education and relevant experience preferred.
  • Experience in a Federally Qualified Health Center (FQHC) environment preferred.
  • Experience supporting Epic Healthy Planet in a population health, community health, or value-based care setting preferred.
  • Experience working with Clinical Operations, Quality, Population Health, Revenue Cycle, and IT teams preferred.
  • Experience participating in Epic upgrades, implementations, optimization projects, and system enhancements preferred.
  • Familiarity with healthcare analytics, quality reporting, and dashboard development preferred.
