Population Health Coding and Documentation Integrity Nurse Auditor

Review clinical records and coding across Pennant’s healthcare operations to strengthen risk adjustment and documentation quality. Partner with providers on education, compliance, and value-based care initiatives.

Description

  • Assess patient records for accurate, complete documentation of health status and chronic conditions
  • Review clinical documentation and coding both before and after care
  • Flag documentation gaps, opportunities for greater specificity, and conditions needing provider clarification
  • Verify ICD-10-CM diagnoses against supporting clinical records
  • Contribute to HCC and other risk-adjustment documentation efforts
  • Audit documentation and coding, then report findings
  • Spot patterns across providers and patient groups
  • Work with physicians, advanced practice clinicians, nurses, coders, and population health colleagues
  • Guide providers on documentation practices, coding specificity, and risk adjustment
  • Create provider training materials, quick-reference sheets, and documentation resources
  • Research coding, documentation, CMS, and regulatory guidance
  • Refine workflows that support accurate clinical documentation
  • Advance quality, compliance, and value-based care initiatives across Pennant

Requirements

  • Hold an active, unrestricted Registered Nurse license
  • Graduate from an accredited nursing program
  • Have at least three years of professional nursing experience
  • Bring strong clinical knowledge, including medical terminology, anatomy, physiology, pathophysiology, and chronic disease
  • Have experience reviewing and interpreting medical records
  • Know ICD-10-CM and clinical documentation principles
  • Apply strong critical-thinking and analytical skills
  • Show exceptional attention to detail
  • Communicate clearly in writing and conversation
  • Work comfortably with physicians and other clinicians
  • Work independently and collaborate effectively across disciplines
