Engagement Care Coordinator - Waverly, Ohio (Onsite)

Coordinate care for hospitalized Ohio youth enrolled in OhioRISE, supporting discharge planning, crisis safety plans, and referrals. Connect families across Southeastern and Central Ohio with behavioral health services and ongoing care coordination.

Description

  • Coordinate immediate care needs for youth hospitalized with behavioral health conditions and enrolled in OhioRISE
  • Guide families through the hospital discharge process
  • Explain the OhioRISE program to families and community partners
  • Refer families to follow-up care and support services
  • Complete engagement milestones through in-person and virtual meetings, including crisis safety plans, assessments, and care plans
  • Schedule and lead Child and Family team meetings through the warm handoff to the youth’s ongoing care coordinator
  • Assess care coordination needs and services with families according to service standards and program outcomes
  • Explore cultural factors that shape family strengths, functioning, and interaction styles
  • Recognize the strengths of children, youth, and families to encourage engagement and healthy outcomes
  • Coordinate family-focused services across homes, schools, and community settings
  • Confirm that care-plan services are appropriate, least restrictive, and aligned with safety and treatment needs
  • Build constructive relationships with children, youth, families, and Child and Family teams
  • Establish partnerships with community resources to address varied family needs
  • Keep documentation current and contribute to program evaluation
  • Participate in team meetings, program planning sessions, and cross-system training
  • Build working knowledge of available community resources
  • Maintain required training, including High Fidelity Wraparound, motivational interviewing, and Cultural Humility
  • Complete other assigned responsibilities

Requirements

  • Three years of experience supporting children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public-sector human services or behavioral healthcare setting with a high school diploma or equivalent; two years with an associate or bachelor’s degree; or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Background in one or more of these areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Cultural humility or responsiveness, supported by training and experience managing complex cases
  • Qualifications and experience serving children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and child-serving systems
  • Strong organization skills and the ability to prioritize and manage multiple tasks
  • Demonstrated cultural awareness
  • Experience supporting individuals and families with multiple needs
  • Broad understanding of community service systems
  • Willingness to participate in and lead cross-system care coordination
  • Strong verbal and written communication skills
  • Ability to work in an internet-based, automated office environment
  • Valid driver’s license required
  • Ability to participate in video calls and meetings in an automated office environment required
  • Ability to maintain a high-speed internet connection of at least 100 Mbps required
