Engagement Care Coordinator – OhioRISE, Ohio Onsite

Coordinate care for Ohio youth hospitalized with behavioral health needs through the OhioRISE program. Support discharge planning, connect families with community resources, and arrange ongoing behavioral health services.

Description

  • Coordinate immediate care needs for youth hospitalized with behavioral health conditions and enrolled in OhioRISE
  • Guide families through hospital discharge planning
  • Explain the OhioRISE program to families and community partners
  • Refer families to follow-up care and support services
  • Participate in in-person and virtual meetings to complete engagement milestones
  • Create crisis safety plans, assessments, and individualized care plans
  • Arrange and lead Child and Family team meetings
  • Provide warm handoffs to youths’ continuing care coordinators
  • Work with families to identify care coordination needs and appropriate services
  • Assess cultural factors affecting strengths, functioning, and family interactions
  • Recognize the strengths of children, youth, and families to promote healthy outcomes
  • Coordinate family-centered services across homes, schools, and community settings
  • Confirm that care-plan services are suitable, least restrictive, and aligned with safety and treatment needs
  • Foster constructive relationships with children, youth, families, and Child and Family teams
  • Establish partnerships with community-based resources
  • Maintain accurate records and contribute to program evaluation
  • Participate in team planning, program meetings, and cross-system training
  • Complete required training in High Fidelity Wraparound, motivational interviewing, and Cultural Humility
  • Carry out other assigned responsibilities

Requirements

  • At least three years supporting children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services with a high school diploma or equivalent; two years with an associate or bachelor’s degree; or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred
  • At least two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Background in family and community systems, community resources, case management, child and family counseling or therapy, child protection, or child development
  • Training and experience managing complex cases with cultural humility and responsiveness
  • Experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and child-serving systems
  • Strong organization skills with the ability to prioritize competing tasks
  • Strong cultural awareness
  • Experience serving individuals and families with multiple needs
  • Broad understanding of community service systems
  • Willingness to participate in and lead cross-system care coordination
  • Strong verbal and written communication skills
  • Ability to work in an internet-based, automated office environment
  • Valid driver’s license required
  • Ability to participate in video calls and meetings through an automated office environment
  • Minimum high-speed internet connection of 100 Mbps required
