Remote Kansas Behavioral Health Care Manager
Remote Care Manager role for Kansas-based professionals supporting Centene members with complex mental and behavioral health needs. Responsibilities include care planning, provider coordination, member education, and helping improve access to cost-effective services.
Tavsif
- Manage complex care activities for members primarily affected by mental and behavioral health needs
- Build individualized care plans and educate members and families about mental health and substance use concerns
- Assess member needs by telephone or during home visits, then recommend or coordinate appropriate services
- Engage members through telephone, digital, home, and other outreach visits
- Maintain continuing care plans for members with high-acuity needs
- Locate suitable providers, specialists, and community resources for mental health and substance use services
- Coordinate communication among members, families, caregivers, community organizations, and care teams
- Track care plans, member conditions, treatment effects, complications, and clinical symptoms
- Support coordinated care and prompt access to required services
- Record member details and care management work accurately
- Explain procedures, provider guidance, treatment choices, referrals, and healthcare benefits to members and families
- Share leadership feedback on ways to enhance care quality and cost efficiency
- Complete additional assigned responsibilities
- Follow applicable policies and professional standards
Talablar
- Candidates must live in Kansas
- Applicants need an active licensed master’s behavioral health credential, such as LCSW, LMSW, LMFT, LMHC, or LPC, or must meet RN requirements with behavioral health experience
- A master’s degree in behavioral health or social work, or a degree from an accredited nursing school, is required
- Two to four years of relevant experience are required
- Working knowledge of behavioral health clinical practice and assessment is required
- Experience in discharge planning, problem-solving, care coordination, computer systems, organizational and time management, and multidisciplinary collaboration is strongly preferred
- Availability is required for either Tuesday through Saturday, 10:00 AM to 6:00 PM, or Sunday through Thursday, 10:00 AM to 6:00 PM
