Apraksts

  • Provide responsive support and direct care to patients and families.
  • Build positive working relationships with colleagues, patients, families, and referral partners.
  • Deliver and oversee care that addresses psychosocial, spiritual, cultural, physical, and biological needs.
  • Respond to clinical questions about Chapters Health System services.
  • Explain the services and their benefits to patients and families.
  • Collect the information and records needed to keep patient files complete, current, and accurate.
  • Record information in tracking systems and the electronic medical record (EMR).
  • Offer reassurance to patients and families.
  • Arrange or conduct in-person visits when requested or needed.
  • Help patients and families reach their desired level of comfort.
  • Draw on appropriate support and specialist resources to address complex situations.
  • Record patient and family contacts in the EMR and update the Interdisciplinary Group (IDG).
  • During emergencies, report to the designated location to maintain service continuity; this may require arriving early or staying overnight.
  • Complete other duties as assigned.

Prasības

  • Hold a current registered nurse (RN) license in the state where you will work.
  • Have at least one year of nursing experience; hospice or hospital experience is preferred.
  • Hold an active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
  • Have prior experience using an electronic medical or health record (EMR/EHR) system.
  • Communicate effectively in spoken and written English; Spanish bilingual ability is preferred.
  • Demonstrate strong clinical judgment, decision-making, and problem-solving skills.
  • Have strong clinical and patient assessment skills.
  • Be able to use equipment with visual and auditory signals.
  • Have a valid driver’s license and automobile insurance in accordance with company policy.
  • Manage time effectively, prioritize demands, and meet patient service standards and deadlines.
  • Handle stressful situations professionally and calmly while providing exceptional customer service.
  • Demonstrate strong patient service skills.
  • Use information-seeking and probing skills.
  • Transfer information accurately.
  • Listen, understand, and respond effectively.
  • Navigate a fast-paced environment and use interpersonal skills to maximize patient visits.
  • Work cooperatively as part of a team.
  • Show urgency in daily work.
  • Use computers proficiently, including MS Office, online research, data entry, and system navigation.
  • Be available for scheduled shifts that accommodate extended operating hours.
  • Perform sedentary work with frequent interruptions.
  • Manage the emotional stress of caring for terminally ill patients and their families.
  • Perform the job’s essential duties and physical requirements, including lifting patients or equipment, bending, pushing or pulling, and kneeling, with or without reasonable accommodation.
  • Satisfactorily complete the position’s competency requirements.
  • Consent to drug and/or alcohol testing after receiving a conditional offer of employment.
  • After a conditional offer, qualify for verification through the Florida Care Provider Background Screening Clearinghouse.
