Remote Oncology Nurse Navigator

Сипаттама

  • Triage oncology patients and provide case management and care navigation, including for patients with possible comorbidities.
  • Deliver direct patient care through virtual platforms, phone, email, and video.
  • Provide clinical guidance and escalate urgent concerns.
  • Review symptom surveys and reported symptoms, support patients, and escalate cases to Nurse Practitioners when needed.
  • Coordinate care, nutritional support, discharge planning, and referrals to providers.
  • Explain diagnoses, Daymark Health’s care offerings, and next steps to patients.
  • Develop trusting relationships with patients and their care networks.
  • Work with Nurse Practitioners, Medical Directors, Social Workers, Clinical Operations, and non-clinical Health Partners.
  • Help address social needs, including access to food and transportation.
  • Follow Daymark Health policies, procedures, clinical protocols, and documentation standards.
  • Use systems, tools, and technology effectively.

Талаптар

  • Bachelor of Science in Nursing degree.
  • Unrestricted compact RN license and licensure in all 50 states.
  • Active, unencumbered multistate RN license under the Nurse Licensure Compact.
  • Additional active RN licenses in non-compact states, including examples such as California, New York, and Illinois.
  • At least three years of nursing experience, including two years in oncology nursing.
  • Commitment to helping patients understand the benefits of palliative care.
  • Oncology certifications such as OCN, AOCN, AOCNS, or CCM are strongly preferred or must be obtained within two years of hire.
  • Comfort with Slack, Zoom, Google Suite, and electronic health record systems.
  • Strong interpersonal, communication, organizational, and adaptability skills.
  • Basic Life Support (BLS) certification is required for patient-facing roles by the start date.
  • Must comply with Daymark Health’s Employee Vaccination and Testing policy.
  • A valid driver’s license is required only for roles that include in-home patient visits.

  • Full-time remote work.
  • Monday–Friday schedule, 8:30 a.m.–5 p.m. in the employee’s local time zone.
  • Coordination of the required Basic Life Support (BLS) certification by the start date if not held at hire.
  • Support for meeting the Employee Vaccination and Testing policy by the start date if incomplete at hire.
