Part-time hospice nurse practitioner role providing patient visits, palliative care, symptom management, and family support in Topeka and Lawrence, Kansas.

Description

  • Conduct routine and urgent face-to-face hospice recertification visits for admissions, transfers, and extended stays
  • Evaluate patient needs, record clinical findings, and work with physicians and the interdisciplinary team to manage symptoms and uphold each Plan of Care
  • Deliver bedside palliative care, patient and family education, and physical and emotional support during crises
  • Triage patients according to acuity and address in-home care needs to help avoid preventable hospitalizations
  • Complete accurate, timely records, clinical documentation, billing, and coding in line with CMS and regulatory requirements
  • Coordinate patient care with the Medical Director, attending physicians, branch leaders, and the interdisciplinary team
  • Train clinical staff and provide continuing education in hospice and palliative care
  • Help create clinical practice guidelines and standards that advance high-quality palliative care

Requirements

  • Hold an active Nurse Practitioner license in the practice state and keep it current under applicable laws and regulations
  • Earn a master’s degree in nursing
  • Hold certification from a national Nurse Practitioner licensing board
  • Have at least one year of clinical nursing experience
  • Maintain active DEA certification where state law permits APPs to prescribe controlled substances
  • Be able to frequently lift, push, pull, and support up to 50 pounds, including repositioning or transferring patients and moving equipment
  • Have a valid driver’s license and acceptable driving record, or reliable transportation for travel between patient homes and assigned work locations
  • At least one year of post-graduate clinical nursing experience in hospice and palliative care is preferred
  • Experience in family medicine, geriatrics, or psychiatry is preferred
