Onsite OhioRISE Care Coordinator in Newark, Ohio

Coordinate OhioRISE services for youth with complex behavioral-health needs. Connect families with providers and community resources across home, school, and community settings in Ohio.

Description

  • Lead and facilitate child and family team meetings from planning through follow-up.
  • Deliver care coordination in families' homes, communities, or other preferred locations.
  • Work with youth, families, natural supports, and service providers to identify needs and appropriate services.
  • Ensure youth and families maintain voice and choice throughout coordinated care and service planning.
  • Keep documentation current to support access to and continuation of OhioRISE benefits.
  • Meet ISBH OhioRISE quality and fidelity standards.
  • Build and maintain knowledge of available community resources.
  • Attend required in-person and virtual training and supervision sessions, including travel as needed.
  • Complete assigned training and maintain all required certifications.
  • Follow confidentiality, professionalism, incident reporting, HIPAA, mandated reporting, and ethical standards.
  • Use databases and electronic health record systems to document client services.
  • Travel throughout the assigned service area.
  • Carry out additional duties as assigned.

Requirements

  • Required experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare: three years with a high school diploma or equivalent, two years with an associate or bachelor's degree, or one year with a master's degree or higher.
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment.
  • Two years of experience in coordinated supportive services or care coordination preferred.
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred.
  • Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development required.
  • Qualifications and experience working with children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and child-serving systems required.
  • Strong verbal and written communication skills required.
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings required.
  • Ability to protect confidential information required.
  • Ability to follow applicable safety rules, regulations, and requirements required.
  • Ability to bill Medicaid without restrictions required.
  • Valid driver's license and ability to operate a motor vehicle required.
  • Appropriate automobile insurance coverage required.
  • Ability to manage deadlines in a fast-paced, high-volume environment required.
  • Ability to work in an Internet-based, automated office environment required.
  • Ability to conduct and participate in on-camera video calls and meetings required.
  • Minimum 100 Mbps high-speed internet connection required.
  • Ability to sit for extended periods while working at a desk and computer required.
  • Ability to lift up to 15 pounds occasionally required.
