OhioRISE Care Coordinator - Onsite Ohio

Coordinate OhioRISE family teams for youth with complex behavioral health needs. Connect families with coordinated services across homes, schools, and community settings.

Description

  • Create and organize family-defined care coordination teams
  • Lead and facilitate child and family team meetings
  • Provide care coordination in families' homes, communities, or preferred locations
  • Partner with youth, families, natural supports, and service providers to identify needs and services
  • Guide teams in defining needs, goals, and action steps
  • Ensure youth and families have voice and choice in coordinated care and services
  • Maintain documentation needed to access and retain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Maintain current knowledge of community resources
  • Attend required in-person and virtual training and supervision, including travel
  • Complete assigned training and maintain required certifications
  • Follow confidentiality, professionalism, incident-reporting, HIPAA, mandated-reporting, and ethical standards
  • Use databases and electronic health record systems for client documentation
  • Travel throughout the assigned service area
  • Perform additional duties as assigned

Requirements

  • Three years of experience providing services or support to children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare with a high school diploma or equivalent; or two years with an associate or bachelor's degree; or one year with a master's degree or higher
  • Experience with family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver's license and ability to operate a motor vehicle
  • Appropriate automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment
  • Ability to work in an internet-based, automated office environment
  • Ability to conduct and participate in on-camera video calls and meetings
  • Minimum 100 Mbps high-speed internet connection
  • Ability to sit for extended periods and perform computer work
  • Ability to lift up to 15 pounds
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification provided upon employment
  • Two years of coordinated supportive services or care coordination experience preferred
  • Experience with autism spectrum disorders and developmental disabilities preferred
