Care Manager Registered Nurse – Westside Chicago Hybrid | CVS Health
CVS Health is seeking a Registered Nurse Care Manager to coordinate appointments, benefits access, education, and healthcare outcomes for members. The hybrid role involves travel throughout Westside Chicago and surrounding areas, with remaining care management work performed from home.
Description
- Apply critical thinking and sound judgment to support the case management process
- Facilitate appropriate healthcare outcomes for members
- Support member appointment scheduling
- Help members identify and access available benefits
- Educate members using care management tools and resources
- Work with members, providers, and community organizations
- Address members’ healthcare needs and social determinants of health
- Travel up to 75% from the home location throughout Westside Chicago and nearby areas
Requirements
- Residency in Illinois
- Reliable transportation and willingness and ability to travel up to 75% of the time from home throughout Westside Chicago and surrounding areas
- Three to five years of clinical practical experience
- Confidence working independently from home while collaborating with teams virtually
- Strong analytical and problem-solving abilities
- Effective communication, organization, and interpersonal skills
- Ability to work independently
- Computer proficiency, including navigating multiple systems and keyboarding
- Proficiency with Microsoft Word, Excel, Outlook, PowerPoint, and proprietary applications
- Associate’s degree
- Active, unencumbered Registered Nurse license in Illinois
- Direct, hardwired internet connection to a modem or router within seven feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload
- Quiet, secure, private home workspace that meets CVS Health and HIPAA requirements
- Ability to work within the state and city of current residence
- Ability to provide workspace furniture
- Comfort setting up and using multiple monitors while navigating multiple applications
- Ability to use email, calendar invitations, Teams messaging, and virtual meetings
- Experience with Microsoft Office 365 or comparable Google Workspace applications
- Ability to access secure systems such as VPN and EHR portals, lock screens, maintain strong passwords, and identify suspicious emails or links
- Ability to resolve common technical issues independently
- Willingness to learn new skills as workplace practices evolve
