Bilingual Spanish RN Case Manager - Medicare Special Needs Program

Provide remote telephonic case management for Alignment Health members enrolled in Medicare Special Needs Plans. Assess complex and chronically ill members, develop care plans, and address gaps in care.

Description

  • Deliver telephonic case management for medically complex and chronically ill Medicare Special Needs Plan members
  • Complete comprehensive health assessments and develop individualized care plans
  • Coordinate services with internal teams, physicians, specialists, and other external partners
  • Teach members and caregivers about disease management and preventive care
  • Track member outcomes and advocate for timely, appropriate interventions
  • Recognize and help resolve service or access barriers that affect care quality
  • Communicate with members and partners by phone, email, and Microsoft Teams
  • Perform essential physical duties such as speaking, hearing, standing, walking, sitting, reaching, handling objects or controls, and occasionally lifting or moving up to 10 pounds

Requirements

  • Hold an active, unrestricted California RN license that is non-compact and valid upon hire
  • Bring at least two years of clinical nursing experience
  • Have a minimum of one year of case management experience
  • Use Microsoft Office applications including Word, Excel, and Outlook proficiently
  • Be willing to obtain RN licenses in Nevada, Arizona, North Carolina, and Texas, with costs reimbursed by the company
  • Be available to work Pacific Time hours
  • Maintain a HIPAA-compliant workspace
  • Bilingual applicants are strongly preferred, particularly those speaking Spanish or Vietnamese
  • Experience with a health plan or IPA is preferred
  • A Bachelor of Science in Nursing is preferred
  • LVNs are not eligible for this position
