OhioRISE Care Coordinator (Onsite)

Coordinate OhioRISE services for young people with complex behavioral health needs. Build family-centered support teams and connect youth to care across home, school, and community settings.

Tavsif

  • Build and organize support teams based on each family’s needs
  • Lead and facilitate meetings for young people and their identified teams
  • Deliver care coordination in families’ homes, communities, or preferred locations
  • Work with young people, families, natural supports, and service providers to identify care needs and services
  • Support positive outcomes for young people and families
  • Maintain documentation required to access and retain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Maintain current knowledge of available community resources
  • Attend required in-person and virtual training and supervision sessions, including travel
  • Complete assigned training on schedule and maintain required certifications
  • Follow ISBH policies for confidentiality, professionalism, and incident reporting
  • Use databases and electronic health record systems to document client information accurately
  • Comply with HIPAA, mandated reporting obligations, and professional ethical standards
  • Travel throughout the assigned service area
  • Complete other duties as assigned

Talablar

  • Professional experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare is required: three years with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination requiring strong organization, efficiency, and attention to detail preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems and resources, case management, child and family counseling or therapy, child protection, or child development required
  • Qualifications and experience working with children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and one or more child-serving systems required
  • Strong verbal and written communication skills required
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings required
  • Ability to protect confidential information required
  • Ability to follow all safety rules, regulations, and requirements required
  • Ability to bill Medicaid without restrictions required
  • Valid driver’s license and ability to operate a motor vehicle required
  • Appropriate automobile insurance coverage required
  • Ability to manage deadlines in a fast-paced, high-volume environment required
  • Ability to work in an internet-based, automated office environment required
  • Ability to conduct and participate in on-camera video calls and meetings in an automated office environment required
  • Ability to maintain a minimum 100 Mbps high-speed internet connection required
  • Ability to sit for extended periods while working at a computer required
  • Ability to lift up to 15 pounds occasionally
