Vytalize Health
Vytalize Health
Vytalize Health works with primary care practices and Accountable Care Organizations (ACOs) to support the shift to value-based care. Its healthcare technology and services combine analytics, care management, and virtual and in-home clinical support, giving independent physicians, group practices, community health centers, and ACOs tools to coordinate care and track performance. The company also supports Medicare-approved remote services for people with chronic conditions and helps participating practices pursue shared savings through value-based contracts.
