Coordinate whole-person care and resources for patients through Providence Medical Foundation’s collaborative care model. Partner with clinical teams, caregivers, and community resources across the care continuum.

Талаптар
- Graduate from an accredited nursing program
- Hold a California Registered Nurse license upon hire
- Bring three years of clinical experience in an HMO, medical group, affiliated model, hospital, or medical office or clinic setting
- A Bachelor’s degree in Nursing or Master’s degree in a related field is preferred
- Certification in Case Management, such as CCM or ACM, is preferred upon hire
- Specialty certification in oncology, rehabilitation, pediatrics, transplant, or wound care is preferred upon hire
- Familiarity with Milliman Care Guidelines is preferred
- Three years of utilization management or case management experience is preferred
- Demonstrate strong verbal and written communication skills
- Ability to coordinate resources across the care continuum
