Remote Telehealth Nurse Practitioner at Carewell

Provide video-based chronic-care, medication, and post-discharge support to Carewell members across the United States. Act as a clinical resource within Carewell’s care navigation program.

Description

  • Conduct video visits for chronic-condition check-ins, medication reconciliation, and follow-up care
  • Evaluate members’ health and social needs, then document clear care plans and next steps
  • Share clinical findings and recommendations with members’ primary care providers
  • Perform follow-up visits after hospital and emergency department discharges
  • Review medications and explain warning signs that require attention
  • Identify readmission risks and coordinate follow-up with the care team
  • Provide clinical guidance to RNs, LCSWs, and care navigators
  • Address clinical escalations on the day they are identified
  • Support training on clinical red flags, medication fundamentals, and escalation criteria
  • Help refine workflows, protocols, and tools as the program develops
  • Complete accurate documentation for every visit on the same day
  • Keep licensure, board certification, and required collaborative practice agreements current
  • Adhere to telehealth, HIPAA privacy, and patient-consent requirements
  • Advance clinical quality, productivity, and compliance KPIs

Requirements

  • Be available during core business hours, Monday through Friday, Eastern Time
  • Live in the United States and be authorized to work there without employer sponsorship
  • Hold Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner board certification
  • Maintain an active, unrestricted APRN license in at least one U.S. state
  • Be willing to obtain additional state licenses as the program expands
  • Bring at least three years of nurse practitioner experience in primary care, internal medicine, geriatrics, or transitional care
  • Explain chronic conditions, medications, and treatment plans clearly to older adults
  • Use telehealth platforms, electronic health records, and care management tools proficiently
  • Work effectively in an evolving environment with ambiguity and rapid iteration
  • Experience with multiple-state licensure, active DEA registration, Medicare or dual-eligible and older-adult populations, telehealth, home-based or transitional care, or bilingual Spanish is preferred
