OhioRISE Care Coordinator – Onsite in Ohio

Coordinate OhioRISE services for young people with complex behavioral health needs. Lead family teams and connect families with community-based support to help them remain together.

Description

  • Plan and facilitate child and family team meetings
  • Deliver care coordination in families’ homes, communities, or other preferred settings
  • Work with youth, families, natural supports, and providers to identify needs and available services
  • Guide teams in defining needs, goals, and actionable next steps
  • Promote youth and family voice and choice throughout coordinated care and service planning
  • Maintain records needed to obtain and retain OhioRISE benefits
  • Comply with ISBH OhioRISE quality and fidelity standards
  • Keep current knowledge of relevant community resources
  • Participate in required in-person and virtual training and supervision, including travel
  • Complete assigned training and maintain required certifications
  • Follow confidentiality, professional conduct, incident reporting, HIPAA, mandated reporting, and ethical standards
  • Record client information in databases and electronic health record systems
  • Travel throughout the assigned service area
  • Handle additional duties as assigned

Requirements

  • At least three years supporting children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public-sector human services or behavioral healthcare setting with a high school diploma or equivalent; two years with an associate or bachelor’s degree; or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided after hire
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Background in family or community systems, community resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, or involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle in homes, schools, and community settings
  • Ability to safeguard confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver’s license and ability to operate a motor vehicle
  • Required automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume setting
  • Ability to work in an internet-based, automated office environment
  • Ability to conduct and participate in on-camera video meetings and calls
  • Minimum internet speed of 100 Mbps
  • Ability to sit for extended periods and work at a computer
  • Ability to lift up to 15 pounds
