  • Assess members' needs and wellness goals to shape appropriate care.
  • Help members and families find suitable care and wellbeing resources.
  • Visit Medicaid members in homes, assisted living, long-term care, and other settings.
  • Spend 75–90% of the time traveling locally.
  • Develop individualized care plans based on member assessments.
  • Support access to services in the least restrictive setting.
  • Design and carry out interventions.
  • Coordinate services and track case management plans against members' personal goals.
  • Work with payers, providers, interdisciplinary teams, and others involved in members' care.
  • Connect members with quality services that support health and wellbeing.
  • Report to the Regional Care Coach Manager.

आवश्यकताएँ

  • Bachelor's degree and at least 2 years of relevant experience in health services, social work, or psychology; or a Florida RN license and at least 2 years of relevant experience; or a Florida LPN license and at least 4 years of relevant experience; or at least 6 years of health services or case management experience as a case worker.
  • Previous experience with Medicare and Medicaid recipients.
  • Residence in northern Pinellas County, Florida.
  • Intermediate to advanced computer skills.
  • Experience using Microsoft Word, Excel, and Outlook.
  • AHCA fingerprinting required.
  • Ability to pass TB screening.
  • Valid state driver's license and proof of personal vehicle liability insurance meeting minimum requirements.
  • Availability to work Monday through Friday, 8:30 a.m. to 5:00 p.m.
  • English and Spanish fluency and ability to pass a language proficiency exam preferred.
  • Community Health Worker experience preferred.
  • Nursing home diversion or long-term care case management experience preferred.
  • Experience documenting case notes electronically and using multiple computer applications or systems preferred.
  • Experience working with older adults preferred.
  • Experience in health promotion, coaching, and wellness preferred.
  • Knowledge of community health and social service agencies and other community resources preferred.
  • 701B Certification preferred.
  • Medication application (RFA) experience preferred.
  • Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse.
