OhioRISE Care Coordinator - Frankfort, Ohio

Support Ohio youth and families through OhioRISE behavioral-health services. Coordinate family teams and community resources while helping reduce hospitalizations.

Tavsif

  • Build and organize teams shaped by each family's needs.
  • Plan, lead, and facilitate child and family team meetings.
  • Deliver care coordination in family homes, community settings, or other preferred locations.
  • Work with youth, families, natural supports, and service providers to identify care coordination needs and services.
  • Support youth in their homes, schools, and communities.
  • Guide teams in defining needs, goals, and action steps.
  • Ensure youth and families retain voice and choice in coordinated care and services.
  • Keep documentation current to secure and maintain OhioRISE benefits.
  • Meet ISBH OhioRISE quality and fidelity standards.
  • Stay informed about available community resources.
  • Attend required training and supervision sessions, including travel.
  • Complete assigned training and keep certifications current.
  • Comply with confidentiality, professionalism, incident reporting, HIPAA, mandated reporting, and ethical standards.
  • Use databases and electronic health record systems to document client services.
  • Travel throughout the assigned service area.
  • Handle other duties as assigned.

Talablar

  • Required experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare: three years with a high school diploma or equivalent, two years with an associate or bachelor's degree, or one year with a master's degree or higher.
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment.
  • Two years of experience in coordinated supportive services or care coordination preferred.
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred.
  • Experience in family and community systems and resources, case management, child and family counseling or therapy, child protection, or child development required.
  • Required qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems.
  • Strong verbal and written communication skills required.
  • Must be able to use a personal or company vehicle to provide services in homes, schools, and community settings.
  • Must be able to protect confidential information.
  • Must follow all safety rules, regulations, and requirements.
  • Must be able to bill Medicaid without restrictions.
  • Valid driver's license and ability to operate a motor vehicle required.
  • Required auto insurance coverage.
  • Ability to manage deadlines in a fast-paced, high-volume setting required.
  • Ability to work in an internet-based, automated office environment required.
  • Ability to participate in and conduct on-camera video calls and meetings required.
  • Minimum high-speed internet connection of 100 Mbps required.
  • Ability to sit for extended periods and work at a computer required.
  • Ability to lift up to 15 pounds occasionally required.
