Advanced Practice Nurse Practitioner
Provide psychiatric assessments and medication management for Harmony Bay patients. Maintain clinical records, prescribe medications, and coordinate care with psychiatric and nursing teams.
Opis
- Complete initial psychiatric assessments within 60 minutes.
- Conduct medication management follow-ups within 20 minutes.
- Finish clinical documentation within 48 hours.
- Submit referrals within 24 hours when patients need a higher level of care.
- Fulfill medical record requests within 30 days.
- Join initial evaluations 10 minutes before their scheduled start and medication management visits within 5 minutes.
- Check the New Jersey Prescription Monitoring Program at least every 90 days when prescribing controlled medications.
- Take part in a one-hour clinical team meeting each week.
- Conduct initial psychiatric evaluations both onsite and virtually.
- Prescribe medications during onsite and virtual follow-up management visits.
- Provide patient refills within 2–3 business days and handle requests for providers who are away.
- Reply to urgent Medical Assistant messages the same day and to non-urgent messages within two business days.
- Coordinate with the Chief Medical Officer, Medical Director, collaborating physician, nursing and clinical leaders, therapists, and psychiatric staff.
- Direct patients to the appropriate level of care.
- Attend scheduled supervision sessions.
- Create and maintain accurate patient records, including intake assessments, progress and treatment notes, termination and contract notes, and required forms.
- Provide staff assistance and coverage as needed.
- Participate in training, orientation, and compliance programs.
Zahteve
- Master’s degree in Nursing and a post-master’s program in an advanced practice role.
- Current, unrestricted Advanced Practice Registered Nurse license.
- At least one year of experience as an Advanced Practice Registered Nurse.
- Current CPR certification.
- PMHNP certification.
- Narcan certification.
- Current Nurse Practitioner certification in the relevant specialty.
- Active DEA registration.
- Active CDS registration if required in the state of employment.
- Computer proficiency, including Microsoft Excel, Word, and PowerPoint.
- Strong computer skills sufficient to use Microsoft Office and electronic health record software.
- Ability to read, write, and spell legibly in English without excessive grammatical or communication errors.
- Strong interpersonal, communication, and organizational skills.
- Ability to balance competing priorities, work under pressure, manage multiple tasks and interruptions, and work independently with flexibility.
- Ability to lift, push, or pull up to 50 pounds and sit, stand, or walk for extended periods.
- Compliance with company policies and procedures and applicable state and federal laws and regulations.
- Ability to provide at least 90 days’ written notice of resignation.
