Build referral partnerships and support patient transitions for a nurse-led home health provider in San Antonio. This role combines healthcare marketing, community relationship management, and patient and referral-source satisfaction.
Descriere
- Serve as the agency’s community liaison for patient-care matters
- Promote compassionate care and positive patient experiences
- Coordinate smooth transitions from hospital to home
- Meet patients before discharge and establish realistic expectations
- Build trusted relationships with patients and their care teams
- Develop and grow referral relationships with hospital case managers, medical assistants, doctors, and other sources
- Document marketing activities and strategic initiatives thoroughly
- Address service concerns while prioritizing patient and referral-source satisfaction
- Work across a blended remote and in-person setting
Cerințe
- High school diploma or GED
- At least two years of healthcare marketing experience; home health experience is preferred
- Knowledge of and compliance with external accreditation standards and applicable federal, state, and local regulations
- Commitment to protecting patient confidentiality and following HIPAA requirements
- Understanding of and adherence to company policies and procedures
