Description

  • Coordinate care activities and facilitate communication among patients, providers, and clinical personnel
  • Organize patient appointments, follow-up visits, and referral workflows
  • Keep patient information complete and current in electronic health record systems
  • Track patient progress while identifying care gaps and scheduling issues early
  • Explain care plans and next steps to patients in a clear, supportive manner
  • Partner with clinical staff to follow established care protocols and regulatory requirements
  • Respond to patient questions, concerns, and escalations with professionalism and empathy

Requirements

  • Experience in patient coordination, clinical coordination, or healthcare administration
  • Working familiarity with EHR or EMR platforms and healthcare management software
  • Solid understanding of medical terminology and clinical processes
  • Clear communicator with strong interpersonal skills and a patient-centered approach
  • Able to manage competing priorities and work independently in a remote setting
  • High school diploma or equivalent
  • Dependable internet access
  • A dedicated, quiet home workspace free from distractions
  • Availability for a 40-hour workweek with rotating shifts from Monday through Sunday
