Onsite Care Coordinator - OhioRISE

Coordinate family-led care teams for Ohio youth with complex behavioral health needs. Connect children and families with community resources and integrated support services.

Description

  • Plan and facilitate child and family team meetings
  • Deliver care coordination in families’ homes, communities, or preferred settings
  • Work with youth, families, natural supports, and service providers to identify coordination needs and services
  • Support youth across home, school, and community environments
  • Guide teams in defining needs, goals, and action steps
  • Ensure youth and families retain voice and choice in coordinated services
  • Maintain records required to access and continue OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Stay informed about available community resources
  • Attend required in-person and virtual training and supervision sessions, including travel
  • Complete assigned training and maintain required certifications
  • Follow confidentiality, professional conduct, and incident-reporting procedures
  • Record client information accurately in databases and electronic health record systems
  • Comply with HIPAA, mandated reporting obligations, and professional ethics requirements
  • Travel throughout the assigned service area
  • Complete additional duties as assigned

Requirements

  • Required experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services: three years with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated support services or care coordination requiring strong organization, efficiency, and attention to detail preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems, resource navigation, case management, child and family counseling or therapy, child protection, or child development required
  • Qualifications and experience serving children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health conditions, and child-serving systems required
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle to deliver services in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety procedures, rules, regulations, and applicable requirements
  • Eligibility to bill Medicaid without restrictions
  • Valid driver’s license and ability to operate a motor vehicle
  • Required automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume setting
  • Ability to work in an internet-based, automated office environment
  • Ability to participate in on-camera video calls and meetings
  • Minimum 100 Mbps high-speed internet connection
  • Ability to sit for extended periods while working at a computer
  • Ability to lift up to 15 pounds occasionally
