Licensed Clinical Social Worker (LCSW) – PRN, Burbank Onsite
Provide individualized neurological rehabilitation to medically complex patients in their homes and communities across Burbank. Assess needs, deliver counseling, and support disability adjustment and behavior change.
Description
- Deliver neurological rehabilitation in patients’ homes and communities
- Evaluate, counsel, and guide medically complex patients
- Create individualized treatment plans
- Support disability adjustment, connect patients with resources, and address behavior modification
- Collaborate with interdisciplinary teams on integrated treatment plans
- Treat patients with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic conditions
- Deliver patient-centered care grounded in evidence
Requirements
- Master’s degree in Social Work
- Active state licensure as a Licensed Clinical Social Worker
- Professional experience as an LCSW
- Strong planning, organization, writing, and time-management abilities
- Clear communication skills with families, professionals, and colleagues
- Ability to accept patients for the full duration of their stay
