- Oversee end-to-end whole-person care solutions for national accounts across oncology, cardiovascular disease, and other conditions.
- Coordinate prevention, screening, early detection, care navigation and management, and Centers of Excellence relationships.
- Advise major accounts on care management, disease management, wellness, network management, and related programs.
- Present programs to support the sales process.
- Recommend program changes to accounts based on collected data.
- Work with internal and external partners to develop and improve condition-specific strategies and integrated care solutions.
- Improve member experience, clinical outcomes, and program effectiveness.
- Partner with providers to expand and strengthen the Centers of Excellence network.
- Build relationships with regional care management groups and improve operational coordination.
- Act as a subject matter expert and train others on care management operations.
आवश्यकताएँ
- Bachelor’s degree in nursing or a related clinical field and at least six years of relevant experience, including program development, marketing, and clinical practice; or an equivalent combination of education and experience.
- Strong project management skills.
- Familiarity with internal utilization management and care management systems.
- Ability to lead productive discussions with internal and external stakeholders.
- Ability to work through ambiguity and adapt quickly to new deliverables and changing priorities.
- Must live within a reasonable commute of a listed work location unless legally required accommodation applies.
- Work in the office three days per week.
- Some patient- or member-facing roles require COVID-19 and influenza vaccination, subject to applicable exceptions.
