Remote Medical Assistant
Provide telephone coaching, health assessments, and care coordination for Medicare patients managing chronic conditions. Support Cadence Health’s clinical AI platform in delivering remote care.
Tavsif
- Contact patients by phone to coach and support those managing type 2 diabetes, hypertension, and cardiovascular disease.
- Guide patients in following personalized care plans through education on behavior change, nutrition, physical activity, and self-management.
- Apply evidence-based approaches, including motivational interviewing and SMART goals.
- Assess patients’ health, lifestyle, nutrition habits, and readiness to make changes.
- Teach patients about disease management, medication adherence, symptom recognition, and prevention.
- Deliver care tasks in line with Cadence clinical policies and protocols, with appropriate clinical supervision.
- Track patient progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.
- Record patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Identify care barriers, including social needs, and connect patients with relevant resources or internal support.
- Coordinate in real time with clinicians and patient support teams using the Cadence platform, Google Workspace, and Slack.
- Share feedback to help improve workflows in a fast-changing environment.
Talablar
- An active Medical Assistant certification from an accredited association is required. Accepted associations include AAMA, AMCA, AMT, NHA, NAHP, NCCT, NRCMA/NRCAMA, and AAH.
- At least five years of experience as a Medical Assistant is required.
- Must be prepared for remote work and have stable, high-speed Wi-Fi.
- Experience in internal medicine, cardiology, nephrology, or care for older adults is preferred.
- Experience educating and supporting patients managing chronic conditions is preferred.
- Spanish fluency is strongly preferred.
- Fluency with AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms, and the ability to discuss credibly with providers and patients where AI can support care and where human judgment is essential.
- Ability to work the specified Monday–Friday schedule in Central, Mountain, Arizona, or Pacific Time.
