Description
- Conduct virtual nursing consultations and provide clinical guidance to patients and care teams.
- Evaluate patient needs, create care plans, and arrange appropriate follow-up.
- Work with physicians, specialists, and interdisciplinary teams to support quality outcomes.
- Record patient interactions, clinical findings, and care recommendations in electronic health records (EHR).
- Teach patients and families about health conditions, medications, and wellness practices.
- Track patient progress and provide continuing telehealth support and case management.
- Follow healthcare regulations, HIPAA requirements, and company policies.
Requirements
- Hold an active RN or LPN license in good standing; Georgia licensure is preferred.
- Have at least two years of clinical nursing experience in a relevant healthcare setting.
- Bring experience with telehealth platforms, EHR systems, and virtual care delivery.
- Demonstrate strong clinical assessment, critical thinking, and patient communication skills.
- Work independently and manage a virtual caseload efficiently.
