Coordinate care, service plans, and case documentation for Nursing Home Transition and Diversion Waiver participants in New York City. This remote role supports continuity of care through communication with participants, families, caregivers, and providers.
Description
- Coordinate care and services for NHTD Waiver participants
- Create, review, and update service plans to reflect participant needs
- Work with participants, families, caregivers, and providers to support continuity of care
- Track service delivery, follow up on open needs, and resolve coordination issues
- Keep accurate case records and documentation that meet program requirements
- Independently manage a caseload with timely communication and follow-through
Requirements
- Direct experience with the NHTD Waiver is required
- Prior Service Coordination experience is preferred
- Knowledge of NHTD regulations, service plans, and care coordination
- Strong communication, organization, and case management skills
- Able to work independently and manage multiple cases
- Reliable, professional, and compassionate approach
- Russian proficiency is a plus
