OhioRISE Engagement Care Coordinator – Bainbridge, Ohio (Onsite)

Support hospitalized Ohio youth and their families through OhioRISE care coordination. Help with discharge planning, safety plans, referrals, and connections to community services.

Description

  • Coordinate immediate care needs for youth hospitalized with behavioral health conditions and enrolled in OhioRISE
  • Guide families through the hospital discharge process
  • Explain the OhioRISE program to families and community partners
  • Arrange referrals for follow-up care and related services
  • Participate in in-person and virtual meetings to complete engagement milestones
  • Create crisis safety plans, assessments, and care plans
  • Schedule and lead Child and Family team meetings through the warm handoff to the youth’s ongoing care coordinator
  • Work with families to identify care coordination needs and available services
  • Assess cultural factors affecting strengths, functioning, and family interaction styles
  • Recognize the strengths of children, youth, and families to promote engagement and positive outcomes
  • Coordinate family-based services across homes, schools, and community settings
  • Confirm that care-plan services are appropriate, least restrictive, and aligned with safety and treatment needs
  • Build constructive relationships with children, youth, families, and Child and Family teams
  • Establish partnerships with community resources
  • Maintain accurate documentation and contribute to program evaluation
  • Participate in team and program planning meetings and cross-systems training
  • Keep required training current
  • Complete other assigned duties

Requirements

  • Three years of experience serving or supporting children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare with a high school diploma or equivalent; two years with an associate or bachelor’s degree; or one year with a master’s degree or higher
  • Knowledge of and experience with Hi-Fidelity Wraparound is preferred
  • Two years of experience in coordinated supportive services or care coordination is preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities is preferred
  • Experience with family and community systems, community resources, case management, child and family counseling or therapy, child protection, or child development
  • Training and experience managing complex cases with cultural humility or responsiveness
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong organizational skills with the ability to prioritize competing tasks
  • High level of cultural awareness
  • Experience supporting individuals and families with multiple needs
  • Broad understanding of community service systems
  • Willingness to participate in and lead cross-systems care coordination
  • Strong verbal and written communication skills
  • Ability to work in an internet-based, automated office environment
  • Valid driver’s license
  • Ability to make and receive video calls and participate in virtual meetings in an automated office environment
  • Minimum high-speed internet connection of 100 Mbps
